The OTC benefit is an over-the-counter spending allowance that many Medicare Advantage plans add at no extra cost. The plan loads a set dollar amount — often each quarter — onto a prepaid card you can spend on approved health products like pain relievers, vitamins, first-aid supplies, and dental care. Allowances and covered items vary by plan, the money usually must be used by the end of each quarter, and you can order in store, online, by phone, or through an app.

What is the OTC benefit?

The over-the-counter (OTC) benefit is a supplemental perk offered by many Medicare Advantage (Part C) plans. The plan gives you an allowance to buy approved non-prescription health items — it does not cost extra and is separate from your prescription drug (Part D) coverage.

Original Medicare does not include an OTC benefit; it is only available through private Medicare Advantage plans, and not every plan offers it.

How the allowance works

Your plan loads a fixed dollar amount onto a benefits card. Most plans reload it quarterly — at the start of January, April, July, and October — though some load monthly.

A key rule: the allowance usually does not roll over. Whatever you don’t spend by the end of the quarter (March, June, September, December) is typically lost, so it pays to use it. Amounts vary widely by plan and region, so check your Summary of Benefits for your exact allowance.

What does it cover?

Eligible items are everyday over-the-counter health products. Common categories include:

  • Pain relievers and cold, cough, flu, and allergy remedies
  • Vitamins, minerals, and supplements
  • First-aid supplies and home health devices (such as blood-pressure monitors)
  • Dental, oral, eye, and ear care
  • Digestive aids and incontinence products

How to order

Depending on your plan and its card administrator, you can usually use the OTC benefit several ways:

  • In store at participating retailers (Walmart, Walgreens, CVS, Dollar General, and others)
  • Online through your plan’s OTC catalog or the administrator’s site
  • By phone through your plan’s OTC order line
  • Via a mobile app such as the OTC Network app

Card administrators you may see

Plans contract with different companies to run the OTC benefit. You may see names like NationsBenefits, OTC Network (InComm), Healthy Benefits+, or a carrier-branded card such as the UnitedHealthcare UCard. The administrator runs the catalog, the card, and the balance — but your plan sets the allowance and the covered items.

Activate before you use it

New cards must usually be activated first — call the number on the card or activate online. If your card is declined, the most common reasons are an inactive card, an item that isn’t on your plan’s approved list, or an allowance that has run out for the quarter.
