Aetna Medicare Extra Benefits Card: OTC, Flex & Food Allowances Explained

Updated June 4, 20268 min readReviewed against medicare.gov

The Aetna Medicare Extra Benefits Card is a debit-style benefit card offered on select Aetna Medicare Advantage plans. Instead of a single "flex" balance, it can hold one or more "wallets," each with its own allowance and rules: an OTC Wallet and CVS OTC Wallet (approved over-the-counter products only), an Extra Supports Wallet (healthy foods, personal care, OTC, utilities, and transportation for members with a qualifying chronic condition), and a Medical Expense Wallet (plan-covered services at provider offices). It is not on every plan, and amounts vary, so confirm what your plan includes in your Annual Notice of Change (ANOC) and Evidence of Coverage (EOC).

What is the Aetna Medicare Extra Benefits Card?

The Aetna Medicare Extra Benefits Card is a benefit card offered on select Aetna Medicare Advantage (MA) plans. It works like a prepaid debit card loaded with one or more spending allowances you can use at approved stores and on approved items.

People often call cards like this a "flex card." Aetna's version is a bit different from a single all-purpose flex balance: the Extra Benefits Card can hold several separate "wallets," and each wallet has its own allowance, its own approved uses, and sometimes its own stores. So your card may carry more than one balance at the same time.

This card is not available on every Aetna plan, and the amounts and wallets vary from plan to plan. Always confirm whether your specific plan includes the card, and what it covers, by checking your Annual Notice of Change (ANOC) and Evidence of Coverage (EOC).

The wallets: OTC, CVS OTC, Extra Supports, and Medical Expense

Each wallet on the Extra Benefits Card has different approved uses and stores. Here is what each one is for:

  • OTC Wallet and CVS OTC Wallet — can be used only to buy approved over-the-counter (OTC) health and wellness products. Examples include cold and flu medicine, allergy medicine, dental care supplies, pain relievers, and first-aid/medical supplies.
  • Extra Supports Wallet — a broader wallet for members who qualify. It covers approved healthy foods, personal care products, OTC products, utilities, and transportation (including public transit and rideshare).
  • Medical Expense Wallet — used at healthcare providers' offices to help pay for plan-covered services.

Which plans include the card and who qualifies

The Extra Benefits Card is offered on select Aetna Medicare Advantage plans, including Dual Eligible Special Needs Plans (D-SNP) and Chronic Condition Special Needs Plans (C-SNP).

For 2026, all Aetna D-SNP members receive a monthly OTC Wallet allowance on the Extra Benefits Card. If a member has a qualifying chronic condition, the OTC Wallet upgrades to an Extra Supports Wallet, which adds spending categories for healthy foods, personal care, transportation, and utilities.

Allowance amounts vary by plan and are not the same for everyone, so this guide does not list a dollar amount. Check your plan documents to see your exact monthly allowance and which wallets you have.

  • Available only on select Aetna MA plans, including D-SNP and C-SNP plans — not all Aetna plans offer it.
  • All 2026 Aetna D-SNP members get a monthly OTC Wallet allowance.
  • A qualifying chronic condition upgrades the OTC Wallet to the broader Extra Supports Wallet.
  • To enroll in any Medicare Advantage plan, you must have both Medicare Part A and Part B. For 2026, the standard Part B premium is $202.90 per month with a $283 annual deductible.

What you can buy (and what you can't)

What you can purchase depends on which wallet the money is in. The OTC wallets are limited to approved over-the-counter health products, while the Extra Supports Wallet adds groceries, personal care, utilities, and transportation.

  • Approved OTC products: cold and flu medicine, allergy medicine, dental care supplies, pain relievers, and first aid/medical supplies.
  • Extra Supports healthy foods: fresh produce, meats, and pantry staples like flour and spices.
  • Extra Supports personal care: items such as soap, shampoo, and conditioner.
  • Extra Supports utilities: natural gas, electricity, and cell phone plans.
  • Extra Supports transportation: public transit and rideshare.
  • Not covered: anything outside your wallet's approved list. OTC-wallet money cannot be spent on groceries, utilities, or transportation — only the Extra Supports Wallet covers those, and only for members who qualify. To see exactly which items qualify, use Aetna's official 2026 over-the-counter product catalog.

How to activate the card, check your balance, and use it

The Extra Benefits Card arrives by mail and must be activated before use, like a debit card. You can activate it using the phone number on the card sticker, a website, or the CVS OTC Health Solutions app (available on Google Play or the App Store).

You can check your balance, view approved products, and place orders by logging in at CVS.com/Aetna or through the CVS OTC Health Solutions app. First-time online users need their Aetna member ID and email address.

  • Where to shop: participating retail stores, online at CVS.com/Aetna, or by phone at 1-844-428-8147. CVS locations inside other stores are not included.
  • Phone ordering hours: a live representative is available 8 AM to 8 PM local time, 7 days a week (excluding federal holidays); the automated system is available 24/7. Online and phone orders ship free.
  • Use it or lose it: the allowance is available on the first day of each benefit period, and any unused balance does NOT carry over to the next benefit period. Plan your purchases before the period ends.

Frequently asked questions

Is the Aetna Extra Benefits Card the same as a flex card?

It is the same general idea — a prepaid benefit card on a Medicare Advantage plan — but Aetna's card can hold several separate "wallets," each with its own allowance and rules, rather than one combined flex balance. Your card may carry an OTC Wallet, CVS OTC Wallet, Extra Supports Wallet, and/or a Medical Expense Wallet, depending on your plan.

Does my unused Aetna OTC allowance roll over?

No. The allowance is available on the first day of each benefit period, and any unused balance does not carry over to the next period. It is a use-it-or-lose-it benefit, so spend it before the period ends.

Can I use the card for food, utilities, or transportation?

Only if you have the Extra Supports Wallet, which covers approved healthy foods, personal care, OTC products, utilities, and transportation (including public transit and rideshare). The plain OTC Wallet and CVS OTC Wallet can be used only for approved over-the-counter health products. The Extra Supports Wallet is for members with a qualifying chronic condition.

Where can I use my Aetna OTC card?

You can use it at participating retail stores, online at CVS.com/Aetna, or by phone at 1-844-428-8147. CVS locations located inside other stores are not included. Phone orders can be placed with a representative from 8 AM to 8 PM local time, 7 days a week, or through the automated system 24/7, and online and phone orders ship free.

Do I need Medicare Part A and Part B to get this card?

Yes. The Extra Benefits Card is a supplemental benefit of an Aetna Medicare Advantage plan, and to enroll in any Medicare Advantage plan you must have both Medicare Part A and Part B. For 2026, the standard Part B premium is $202.90 per month with a $283 annual deductible.

How do I find out if my plan includes the card and how much I get?

Allowances and wallets vary by plan, so check your Annual Notice of Change (ANOC) and Evidence of Coverage (EOC), or log in at CVS.com/Aetna or the CVS OTC Health Solutions app to view your current balance and approved products.
